1. DECISIONS MADE — bullet list, exact wording where it matters.
2. ACTIONS — table: action | owner (from my team list, or "CLIENT" or "UNASSIGNED") | deadline (explicit if stated, otherwise propose one and mark it "proposed").
3. OPEN QUESTIONS — anything discussed but not resolved.
4. A follow-up email to attendees: 3-sentence summary + the actions table, ready to send.
5. RISK FLAG — one thing in this meeting most likely to be forgotten or go wrong.
Do not invent decisions that weren't made. Ambiguous items go to OPEN QUESTIONS, not ACTIONS.
